|

|
The Community-Based Long-Term Care Program provides for
ongoing coordinated care by a professional team in a community setting.
|

Benefits and Services
|
|
|
Health and Community Services staff assess, treat, monitor and
discharge clients, as appropriate. (Example of care: wound care management,
and follow-up after cardiac/stroke rehabilitation. The duration of the
service may extend over long periods of time.

Brief Description of the Policy Issue

To maintain adults in the care setting of choice for as long
as possible. Enable adults with chronic health care or debilitating
conditions to receive treatment or have case monitoring in a community
setting where possible.

Ensures the development of a comprehensive care plan, prevents
unnecessary visits to physicians and hospitals, reduces hospital stays, and
allows the individual to recouperate at home.

Individuals with chronic health care or debilitating
conditions who are homebound and require assessment, intervention, or
ongoing monitoring by health and community services professional staff.

Brief description of any factors that readers should consider
|
|
|
Care is usually given in private homes. Other settings include
health clinics, boarding homes, alternate family care homes and personal
care homes. Services are provided by professional staff of the Regional
Health and Community Services Boards including: registered nurses,
physiotherapists, occupational therapists, social workers and lab
personnel, where services are available. This program is funded by the
Department of Health and Community Services through global funding to
Regional Health and Community Services Boards which manage these programs.
This service is not age specific. Professional services are part of these
Boards' operational and capital budgeting.
